Recovery guide · day-by-day
Brazilian butt lift (BBL) recovery: week by week
BBL recovery is unusual: avoidance of direct pressure on the buttocks for 2-4 weeks is essential to fat survival, and compression garment on liposuction donor sites is worn for 6 weeks. Most patients return to desk work at 1-2 weeks with a special cushion.
★Key takeaways

Day 0-3
What happens
- Overnight hospital stay
- Compression garment on lipo donor sites
- No direct pressure on buttocks
- Significant bruising on lipo sites
Do
- Sleep on stomach or side
- BBL pillow for any sitting
- Walking encouraged (DVT prevention)
Do not
- Sit directly on buttocks (must use BBL pillow under thighs only)
- Sleep on back without pillow elevating buttocks
Call surgeon if
- Calf pain, shortness of breath (DVT/PE risk)
- Sudden severe pain unrelieved by prescribed pain medication
- Calf pain, swelling or shortness of breath (possible DVT or pulmonary embolism)
- Spreading redness, hot skin, fever over 38°C (possible infection)
- Heavy bleeding through dressings
- Sudden change in colour of skin or tissue (white, blue, or grey)
- Persistent vomiting that prevents you taking medications
Week 1
What happens
- Bruising peaks then fades
- Compression garment 24/7
- Walk only / no sitting on buttocks
Do
- BBL pillow for car / desk
- Sleep on stomach
- Walk frequently
Do not
- Direct buttock pressure
- Hot showers (heat impairs fat survival)
Call surgeon if
- Sudden severe pain unrelieved by prescribed pain medication
- Calf pain, swelling or shortness of breath (possible DVT or pulmonary embolism)
- Spreading redness, hot skin, fever over 38°C (possible infection)
- Heavy bleeding through dressings
- Sudden change in colour of skin or tissue (white, blue, or grey)
- Persistent vomiting that prevents you taking medications
Week 2-4
What happens
- Sitting allowed with BBL pillow only
- Significant fat resorption (30-50%) starts to be visible
- Compression continues
Do
- Standing meetings
- Short walks frequently
Do not
- Sit on regular chair, ride bicycle, drive long distances
Call surgeon if
- Sudden severe pain unrelieved by prescribed pain medication
- Calf pain, swelling or shortness of breath (possible DVT or pulmonary embolism)
- Spreading redness, hot skin, fever over 38°C (possible infection)
- Heavy bleeding through dressings
- Sudden change in colour of skin or tissue (white, blue, or grey)
- Persistent vomiting that prevents you taking medications
Week 4-6
What happens
- Light cardio resumes
- Sitting more tolerable
- Continue compression on lipo sites
Do
- Resume short cardio (treadmill walking)
- Continue BBL pillow for prolonged sitting
Do not
- Glute exercises
Call surgeon if
- Significant asymmetry or palpable lumps
Week 6-12
What happens
- Final fat volume settles by 3 months
- Resume full activity at 8-12 weeks
- Final contour visible
Do
- Resume gym including glutes from week 8-12
- Maintain stable weight (weight loss shrinks fat transfer)
Do not
- Lose weight in first 6 months (fat is still settling)
Call surgeon if
- Persistent asymmetry, palpable hard areas
Common questions
Brazilian butt lift (BBL) recovery - common questions
How long do I have to avoid sitting?
Most surgeons say no direct buttock pressure for 2-4 weeks. After that, sitting with a BBL pillow (which supports the thighs and ischial tuberosities, suspending the buttocks above the seat) is allowed. Regular sitting typically resumes at 4-6 weeks but BBL pillow recommended for long sitting through 8 weeks.
How much fat survives?
Approximately 50-70% of injected fat survives long-term. The remaining 30-50% is resorbed in the first 3 months and is permanently lost. Your surgeon overcorrects to account for this. Smoking and weight loss after surgery reduce fat survival further.
When will I see the final result?
Significant volume loss visible in first 3 months as resorption occurs. Final shape at 3-6 months. Surviving fat is permanent at that point.
How do I sleep after a BBL?
Sleeping on stomach for first 4 weeks. Some patients use a body pillow to maintain stomach-sleeping. Side-sleeping with a pillow under hips to lift the buttocks off the bed is acceptable. Sleeping on back is avoided in first 4 weeks because it places direct pressure on the buttocks.
Is BBL safe in Australia?
Australian BBL is performed in accredited hospitals with intraoperative ultrasound guidance (AHPRA 2023 requirement). This is dramatically safer than overseas (Mexico, Turkey) where office-based theatres without ultrasound have published mortality rates around 1 in 1,000. Australian published rates are substantially lower but BBL remains the highest-mortality cosmetic procedure. Only consider with FRACS plastic surgeon in accredited hospital.
